What is Transition Team? A familiar, dementia-trained caregiver providing individual support and helping your family member adjust to a new home

Memory loss and other dementia symptoms affect every person differently, so it is important to have a specially trained caregiver get to know the senior and family, learning their history, routines and needs. At American River we focus on understanding each individual, which allows us to provide fluid and flexible dementia care as the disease progresses.

Aging is a journey with many steps along  the way, and each one can require unique and challenging decisions. For a family living with a senior whose needs are changing, including memory loss and dementia, it’s often hard to know when to take the next step and what the right step is, especially when it comes time to consider out-of-home care.  American River Home Care and Laura Wayman can help.

Utilizing the expertise of Laura Wayman, American River created the Dementia Transitions Team to help families in need deal with the emotional and stressful evolution of care necessary for individuals with dementia. Tune in and find out more about this special and much needed program...
