Making a Real Universe

Have you ever been thrown out of a story because the universe the story is in feels . . . fake, campy, unrealistic with characters who would never do what they are doing? It happens and it is something we writers never want to happen. How do we prevent it? How do we create fictional universes for mystery, fantasy, science fiction, or any other genre and keep them feeling real?
