As the number of Americans who need long-term care continues to grow, they are constantly transitioning between health issues and care environments, providers and needs. When programs focus only on elements of care instead of people, no one has a full picture of the participant’s health – leading to suboptimal care and frustration for caregivers, family members and program managers.
