What is the Greatest Cause for Miscommunication?

Only 1 in 4 is able to demonstrate their ability to use Facts rather than Opinions to state their case. Not easy for many to digest.

There is a critical gap, or confusion for miscommunication - believing something to be what it is not, with the worst case of this being our failure to even recognize our own mistake and blindness in the matter.
