    Insurance Reimbursement Rates for Nurse Practitioners

    We discuss the recent development of insurers lowering the reimbursement rates for NPs to 85% of the physician rate. Our guests this month are Tom Hampton and Bob Smithing ARNP.

    Tom Hampton, the former Commissioner for the DC Department of Insurance, Securities and Banking, a role in which, most recently, he was responsible for providing oversight and direction of the agency that regulates all financial services industries in the District of Columbia.
    Tom has advised clients on regulatory approvals required in insurance agency, insurance company, captive insurance and risk retention group licensing issues. He has assisted clients with captive insurance company formation and licensing issues. He has also advised clients with life and health policy form filings. Tom has advised a client that is coordinating with state insurance departments and state health departments on the establishment of their state health insurance exchange, as required under the Affordable Care Act.

    Tom has advised clients on financial regulatory matters, including NAIC examination procedure concerns, accounting practices and procedures issues, and annual statement filing concerns. Tom also assisted clients with market conduct examination concerns and other market conduct regulatory issues.

    Bob has been a Family Nurse Practitioner since his graduation from the University of Pennsylvania in 1981. He was granted a Master of Science in Nursing degree upon graduation and was subsequently certified by the American Nurses Credentialing Center as a Family Nurse Practitioner. He is a past president of the American Academy of Nurse Practitioners and served on the original board of the American College of Nurse Practitioners.

    A Proper ROBS Structure:

    ROBS arrangements typically involve the following sequential steps:

    Establish a new corporation;
    The corporation hires the entrepreneur as an employee;
    The corporation adopts a 401(k) plan that specifically permits plan participants to direct the investment of their plan accounts into a selection of investment options, including employer stock;
    The entrepreneur as employee initiates a direct rollover or trustee-to-trustee transfer of retirement funds from another qualified retirement plan into the newly established plan;
    The entrepreneur as employee then directs the investment of his or her 401(k) plan account to purchase the corporation’s stock at par value;
    The entrepreneur personally acquires corporate stock in exchange for direct investments into the corporation, assets transferred to the corporation, or as reimbursement for start-up costs; and
    The company utilizes the proceeds from the sale of stock to purchase an existing business, fund an existing business, or begin a new venture.

    A wise entrepreneur will understand what the promoter is doing for them versus what they still need to do for themselves. In general, a ROBS promoter creates the corporation, registers the Plan, and provides documentation to their clients that cover the adoption of the Plan by the company and issuance of the stock to the entrepreneur and the Plan. Since the client is, typically, the Plan Trustee, the Board and Officers of the Corporation, the excuse “I thought they were going to do that” will not work.

    Segment 9: Liberty Health Share Members See Reduction in Costs. But Why?

    We’re talking about Liberty Health Share.  Again this is not health insurance. It is medical cost sharing.  And it is allowable under the Affordable Care Act.  I’ll admit medical cost sharing is difficult to understand since we have lived with medical insurance for so long.  But share amount members have paid actually dropped by 50-dollars a month last year.  How could this happen in this environment?  I put that question to Liberty Health Share Executive Director Dale Bellis.  He told me three reasons.

    Number one we actually share in the actual numbers…. Are less there as well.

    Jim Hone, Santa Monica Fire Chief (RET): Northridge after 20 years

    Our guest this morning is Jim Hone.

    Jim is the former Santa Monica Fire Chief and has been involved in emergency and disaster response, recovery and training for more than 36 years.

    Today, Jim is going to discuss Earthquake preparedness, response and recovery - as April is Earthquake Awareness Month - and in remembrance of the great quake of 1906 in San Francisco and our own Northridge quake 20 years ago.

    Jim will share his experiences in Santa Monica following the Northridge EQ and the effects of more than $250 million in damages and more than 19.5 years of recovery before closing the reimbursement books. 

    Jim is the former Santa Monica Fire Chief and has been involved in emergency and disaster response, recovery and training for more than 36 years.

    Jim served on several local, state and federal disaster response teams and committees. He has responded to six Federal Disasters including the 1995 Oklahoma City Bombing and the 2001 World 

    Section Chief at the Santa Monica Farmers’ Market Multi-Casualty Incident involving 79 victims including 10 fatalities. 

    Jim is also a regular guest on this show and we’re always thrilled to chat with him.

    Jay: "There's a common transparency theme with the NSA and CMS (Centers for Medicare and Medicaid Services).  There's the question of what missions are being served--neither seems primarily focused on the general citizenry, and acts in disdain, at best, for democratic processes.  Both topics require explanation--most people haven't heard of CMS or know the role it plays in price fixing. Likewise, while we know the NSA has been collecting exploits and back doors rather than publicizing and fixing them, that's something else most people are unaware of."

    The Centers for Medicare and Medicaid Services.  They set Medicare and Medicaid reimbursement rates, which are the basis for most insurance reimbursement rates.
