RACE/ETHNICITY - Check only one box using the following definitions: White (not of Hispanic or Latino origin) Hispanic or Latino Black or African American (not of Hispanic or Latino origin) American Indian or Alaskan Native Asian (not of Hispanic or Latino origin) Native Hawaiian or Other Pacific Islander (not of Hispanic or Latino origin) Two or more races (not of Hispanic or Latino origin) This is a typical example of questions asked on an employment form. And it does not stop there. Recently, we had a loved one pass away and there it was again, the only question regarding race was "are you Hispanic?"
