Both Dan and Jim recently announced their retirements from their respective EMS leader positions.  With over 70 years of combined EMS experience, they offered an excellent perspective on advancing your EMS career into a leader and manager position.  Their recommendations included working with a mentor, advancing your career, leadership and interviewing suggestions and defining company culture. Dan has over 40 years experience in EMS and recently retired from Emergency Services of Door County where he was director for over 30 years.  Dan also was State of Wisconsin EMS Section Chief, Director at MedFlight in Madison and held previous positions with Flight For Life and Paratech Ambulance, both in Milwaukee.  For fourteen years, Dan was chairman of Wisconsin's EMS Advisory Board.  He continues as conference director for Paramedic Systems of Wisconsin Conference and as an EMS consultant. Jim recently concluded a 31-year career with Flight For Life, Milwaukee.  He was hired in 1986 as a flight nurse and in 1991 was promoted to chief flight nurse.  Jim transitioned to System Transport Director in 2001, a position he held until his retirement.  Jim was responsible to purchase new aircraft, made significant upgrades to the fleet and managed a system that completed over 33,000 safe flights. The former billing manager was the whistleblower in this case and reported the wrongdoing. MedStar Ambulance is alleged to have: (1) billed for transports that were not medically reasonable and necessary; (2) billed for higher levels of services than patients’ conditions necessitated; and (3) billed for higher levels of services than were actually provided. Besides the MedStar case, we also discussed how to reduce an ambulance service's Fraudulent Claims Act risk with internal policy and procedures. Recently, LifeStar EMS created an advanced internal critical care level that requires an advanced board certification (FP-C, CCP-C) . Their goal is to have their critical care paramedics meet all the same requirements as helicopter EMS services like Flight For Life and Med Flight. These are advanced certifications through the Board of Critical Care Transport Paramedic Certification (BCCTPC); exams are extremely tough and have a 20% pass rate. HIPAA compliance has moved well past simply handing the patient a HIPAA compliance brochure explaining their rights and getting their signature.  HIPAA stands for Health Insurance Portability and Accountability Act. Conducting a periodic HIPAA Security Risk Analysis is definitely a required mandate of HIPAA compliance for ambulance services as the organization and its employees and volunteers come in contact with ePHI (electronic patient health information). The developing best practice is to complete a Security Risk Analysis on an annual basis (or after any significant change in your technology environment) due to some relatively recent pronouncements that apply to those organizations who need to comply with ‘meaningful use’ reporting requirements. Currently, one in five Wisconsin residents is on state funded Medicaid. The state's Medicaid budget is $9.1 billion. Without Wisconsin Department of Health Services increasing the reimbursement rate or industry stakeholders pursuing legislative action, growing budget gaps caused by low Medicaid payments will continue to plague the industry and lead to a greater cost shift to other payers, local property taxes or potentially result in a reduction of services. At 20 minutes into the program State of Wisconsin EMS and Trauma Medical Director Dr. Suzanne Martens joined us to explain House of Representatives bill 4365, Protecting Patient Access to Emergency Medication Act.  It has been recognized that EMS technically operates outside of the current DEA rules for controlled substances for pain and seizures.  The DEA has allowed this to continue, but has stated it cannot do so without a change in the actual regulations.  A draft bill to change these regulations has been written and proposed.  The House of Representatives is processing it, and will vote in the near future.  Without this bill change, all administration of controlled substances will require radio/phone contact for an order with medical control; and the ordering and stocking of these medications will be complex and costly, with potentially every ambulance being considered an "office".  Since the majority of requests for paramedic intercepts involves controlled substances, this impacts all levels of care.  >> Click for HR4365 Toolkit and how to contact your local elected federal representative.
