It is an innovative, unique program for adolescents and young adults who recently have had unusual thoughts and behaviors or who have started hearing or seeing things that others do not. OnTrackNY / Connections helps people achieve their goals for school, work, and relationships. Its mission is to provide premier, evidence based services such as psychiatric treatment, employment and educational support, substance abuse treatment, and family education and support, for people and their families. The clinic offers care from a specialized team for up to 2 years, based on clients’ needs and preferences. The team consists of a psychiatrist, nurse, social workers, mental health clinicians and an employment specialist, many of whom also have appointment at the Columbia University Medical School.
