(1)   All medical records pertaining to care received by Deaysia Thomas-Daniels at any Temple University Hospital Department April 6, 2010 through today's scheduled visit April 12, 2010; including but not limited to doctors' examination notes, Xrays, films, sonograms, tests ordered, injections, medications given or prescribed, nurses notes, intake and discharge instructions, medications, emergency room records; follow up examinations, diagnoses, prognoses, etc.

We understand there is a copying charge for these records and ask that you contact Roxanne Grinage, HireLyrics Administrative Services, who is documenting Federal Official Corruption Fraud Civil Rights cause of action against two criminally behaving City of Philadelphia Department of Human Services social workers, Lakeisha Kenyetha and L. McMillian as managed by DHS Commissioner, Anne Marie Ambrose regarding recorded misconducts and violations of CPS rules policies and procedures which willfully recklessly endanger the best interests of our daughter Deaysia Thomas-Daniels.  Please telephone, email or postal mail cost of copying records to our administrative service and costs of producing Deaysia Thomas-Daniels Temple University Hospital Medical Records 04/06/10 through 04/12/2010 will be paid promptly:
