The program requires members to complete an e-PHR and PECAN Risk Evaluation, which offers a complete examination of a person’s Stress Level, Fitness & Nutrition, Weight Management, Heart Health and Cancer Risks. Support and Consultation services are structured as six-month to multiple-year relationships that focus on overcoming barriers and achieving measurable goals.
